General Instructions:

  1. After installation of DISE capsule, data of all the employees (of Class A, B and C only) of the concerned department must be entered in DISE capsule. All the fields are mandatory to be filled.
  2. Export the entered data in CD and write the name of department/office on CD clearly.
  3. List of Class 4 employees / Drivers should be entered in prescribed performa (Excel Format available for download on district Malerkotla website and must be submitted in the CD along with DISE Capsule data.
  4. The physical copy of the following documents must be submitted at NIC office Malerkotla:
    1. Declaration certificate by HOD
    2. Summary of entered records.
    3. Checklist Signed by HOD.
    4. CD in which the data entered.
    5. Govt. approved relevant documents of individual employee who is marked Disable /Pregnancy /Lactating woman /On Long leave/BLO/medical certificate of serious Chronic Patients etc. in DISE capsule. (The documents are very important to be submitted in case of election duty exemption)

Note:      1. Documents mentioned at point 1, 2 and 3 are available under REPORTS tab in DISE capsule.

2. Address of NIC office: Room No. 10, Network/Server room

                                              National Informatics Centre (NIC),

                                              DC office, opposite verka booth, Malerkotla.

3. Data of Class 4/Drivers/Security Guards must NOT be entered in DISE capsule.
