Important Instructions

1. Data should be entered carefully, completely, and correctly in all the fields.
2. It is the responsibility of the Head of the department/office to submit the complete/accurate data of all the employee on this portal in all respects.
3. Data should be finalized only after the complete data entered on the portal in all respects and then print the undertaking certificate and employee checklist.
4. Declaration Certificate should be signed and stamped by Head of the Department.
5. Relevant Supporting documents (signed and stamped by Head of the Department) of the employees those are marked during data entries mentioned below should also be submitted along with Undertaking and checklist.
   1. Differently Abled
   2.  On Long Leave
   3. Expecting Mother
   4. Lactating Mother (Baby Age)
   5. Heart Patient
   6. Cancer Patient
   7. Severe Chronic Disease
   8. Double Duty
   9. BLO
   10. Sector Officer
6. Data of mentioned below should not be entered on the portal.
   1. Medical Staff like Medical Officer
   2. Veterinary doctors
   3. Nurses
   4. Para Medical
7. Data of below class 4 Employees  should not be entered on the portal.
   1.  Peon
   2. Sweeper
   3. Beldar
   4. Gardener – Etc
8. Data of drivers/security guards should also not be entered on the portal.
